A new two-year Parkinson’s Disease (PD) community care programme has been launched by the National Neuroscience Institute, Singapore General Hospital (SGH) and Temasek Foundation, to provide support for caregivers of SGH patients with PD — for free.

Trained Community Care nurses from SGH who are supported by a multidisciplinary team will provide care and assistance:

  •   Symptoms assessment
  •   Care coordination
  •   Early intervention
  •   Simple targeted exercises
  •   Caregiver training

The service is provided at Community Nurse Posts located at Senior Activity Centres, or patients’ homes.

Who can apply for this programme?

SGH patients with moderate to advanced stages of PD and their caregivers living in Bukit Merah, Telok Blangah, Katong, Chinatown and Tiong Bahru may apply through their doctors.
